Case Management – Austin

This program provides case management. Case management is a core service for Vivent Health, directly addressing the agency’s mission to respond to the HIV needs of the Austin area. Case management is a time-limited intervention that seeks to assist persons who have difficulty accessing or remaining in primary medical care and for delivering and coordinating HIV and other supportive services. Main Services: help pay for healthcare, help find housing, transportation, daily life skills, medical care, HIV treatment, mental health care, financial assistance, support groups, more education representation.
