Community Resource Center at Hornsby Bend Health & Wellness Center

Central Health resource center has a dedicated CHW to help patient/clients navigate non-clinical community resources or services determined by eligibility criteria. No appointment is necessary for diapers or feminine products. Clients can receive supplies once a month. If first time client, please bring the birth certificate of each child. While supplies last. Main Services: help pay for food, baby supplies, diapers & formula, books, help pay for healthcare.
